What is the Primary Care Mental Health service?

Direct Local Health provides weekday triage appointments with a mental health practitioner, leading to telephone or face to face appointments with a Community Mental Health Worker and/or appointments with a Community Support Worker as appropriate.

GP Practices if appropriate add patients to a triage clinic, with patients then contacted on the day of the triage clinic by a Community Mental Health Worker to review a case plan and decide upon on-going support plans.
